Iowa health insurance for 2027
A Farm Bureau health plan isn't insurance under Iowa law, and it isn't an ACA plan. Every applicant answers health questions, you have to be a Farm Bureau member, and it comes with no premium tax credit. A marketplace plan can't turn you down or charge you more for a pre-existing condition.

Farm Bureau health plans and ACA plans
These plans sit outside Iowa's insurance code, and they work differently from marketplace coverage.
Not insurance under Iowa law
Iowa Code 505.20 exempts health plans from a qualifying Iowa nonprofit agricultural organization from the state insurance code. The organization files a certification with the insurance commissioner, and a registered third-party administrator runs the plan.
Underwriting
Every applicant to the Farm Bureau plan is medically underwritten. Marketplace plans can't refuse you, or charge you more, because of a pre-existing condition.
No tax credit
Premium tax credits apply only to coverage bought through the marketplace. With a Farm Bureau plan you pay the whole premium, whatever your income.
Who can apply
You must live in Iowa and be a Farm Bureau member to apply.
What's changing in Iowa for 2027
Five insurers filed 2027 plans in Iowa. For 2026, 123,304 Iowans picked a marketplace plan, down 9.9% from 2025.
If your insurer is leaving
Choose another plan by December 15 to start January 1. If you don't, HealthCare.gov can match you to a similar plan from another company, so make sure your doctors are in it before January.
Rate requests from 4.4% to 16.84%
Insurers asked for average 2027 increases from 4.4% to 16.84%, depending on the insurer, before any tax credit. The Insurance Division hasn't posted final rates yet.
Iowa Medicaid changes in 2026 and 2027
Iowa expanded Medicaid through the Iowa Health and Wellness Plan on January 1, 2014. Its new rules start on December 1, 2026.
New applicants from December 1, 2026
Adults who apply for the Iowa Health and Wellness Plan on or after December 1, 2026 must meet monthly community engagement requirements, such as work, job training, education or volunteering, unless they're exempt.
Current members at renewal
Adults already in the Iowa Health and Wellness Plan must meet the requirements at their renewal, starting with February 2027 renewals.
Renewals every six months
Most members move to 6-month renewals at their next renewal, starting with March 2027 renewals.
For Iowa employers
For an Iowa business, an ICHRA (CMS has renamed it a CHOICE arrangement) pays employees back tax-free for the individual health insurance plans they buy.

Questions from Iowans
Is a Farm Bureau health plan the same as ACA insurance?
No. Iowa law says the Farm Bureau plan isn't insurance. It isn't an ACA plan, it underwrites every applicant, and it has no premium tax credit.
What if my insurer stops selling Iowa marketplace plans?
Pick another plan by December 15 for coverage from January 1. Otherwise HealthCare.gov may place you in a similar plan, which may use other doctors.
When do Iowa Medicaid work requirements start?
December 1, 2026, for new applicants. Current members must meet them starting with February 2027 renewals.
