North Carolina health insurance for 2027

Two things change in North Carolina for 2027. Insurers have asked for higher rates, and NC Medicaid adds work rules and six-month renewals on January 1, 2027. Open enrollment on HealthCare.gov runs November 1, 2026 to January 15, 2027.

What's changing in North Carolina for 2027

North Carolina had the largest percentage drop in marketplace sign-ups of any state for 2026: 761,457 people picked a plan, down 21.9% from 2025.

Rate requests by insurer

Five returning insurers filed increases from 8.5% to 27.41% for 2027, and a new insurer filed plans sold off the exchange. The Department of Insurance hasn't posted final rates yet.

If your insurer leaves

If your insurer stops offering plans and you don't choose, HealthCare.gov may put you in a similar plan from a different insurer. Picking your own by December 15 keeps the decision about your doctors with you.

Who filed plans for 2027

Six insurers filed individual plans for 2027: five returning ones and a new one whose plans are sold only off the exchange, not on HealthCare.gov.

NC Medicaid changes in 2027

North Carolina expanded Medicaid on December 1, 2023, for adults 19 to 64. On January 1, 2027, two rules change for most of them.

Work and community engagement

Most adults 19 to 64 will need 80 hours a month of work, school, volunteering or another approved activity, or $580 a month in earnings, unless they're exempt.

Renewals every six months

Most of these adults will renew every 6 months instead of once a year.

The state's trigger law

North Carolina law ends the expansion if the federal government's share of its cost falls below 90%.

For North Carolina employers

In North Carolina, an employer can offer an ICHRA, now also known as a CHOICE arrangement. Each employee picks an individual plan, and the business pays them back tax-free, to a limit it sets.

Questions from North Carolinians

What if my insurer leaves North Carolina's individual market?

Choose a new plan by December 15 for a January 1 start. If you don't, HealthCare.gov may choose a similar one for you, and it may not include your doctors.

How much are North Carolina premiums going up for 2027?

Returning insurers asked for increases between 8.5% and 27.41%, depending on the insurer. Those are requests, and final rates come later. What you'll pay also turns on your plan, your age, where you live and any tax credit.

Does NC Medicaid have work requirements in 2027?

Yes, from January 1, 2027, for most adults 19 to 64. Parents of children under 14, pregnant people and people with disabilities are among those exempt.
